those are two problems...the floor is also too cluttered.Could have died from fall damage or gotten scraped from the substrate.
That's a total non-issue with terrestrials kept predominantly dry.lack of ventilation
how can you telll the light is a heat lamp?not to mention the heat produced by the light.

Are wood chips for certain reptiles?? This isn’t the first time I’ve seen someone use them for tarantulas without knowing any better. One T I got from my neighbors, aunt had wood chips, I had to remove .Just to add, the substrate is definitely not appropriate for a tarantula. Dirt only!

There's no way to know exactly why she died. Most of the time if a tarantula isn't doing well there isn't a way to tell just from looking. All you can do is, if you want to, try again with some more online research and recommendations from people with more firsthand experience.
The people on here, although they can often be very harsh, genuinely care for tarantulas and want them to live happy, long and, comfortable lives. Try not to disregard advice due to lack of tact as it's usually good.
There is a search feature at the top right of the website (little magnifying glass) where you can look up specific species, enclosures, care info etc. I would recommend looking at what's working for other people and starting from the ground up.
I hope you're able to get past this (it's never easy losing a pet) and continue in the hobby
